Saint Joseph Health Systems (SJHS) is hosting a clinic to offer pre‐participation physical exams for local middle and high school students at Medical Office Building # 2 on Lake Avenue. Physicians of the Saint Joseph Physician Network will be performing the physicals on Tuesday, May 24th from 5 to 8 p.m.
Students should wear comfortable clothing and bring their ID. There is a new physical form that is required by the IHSAA for completion of the physical. The first page of the form must be competed and all required signatures be done prior to the exam. All students who arrive prior to 7:45 are guaranteed to be seen for a physical.
There is a $20 fee per student physical. Registration is not necessary.
